Emily’s PAH severity is concerning. Her right heart function is severely impaired, with CO/CI at severe levels. Her NT-proBNP indicates right heart failure. Her risk scores are very high and her disease is having a profound adverse effect on her life.
Treatment Plan
- Educate patient on her progressive disease and the importance of an aggressive treatment approach
- Educate patient on risk assessments and the importance of lowering her risk score and set goals along the treatment journey
- Initiate parenteral prostacyclin and background therapy, uptitrate the dose as rapidly as tolerated
- Counsel patient and family on managing continuous pump therapy, expected adverse events, and possible AE mitigation strategies
