Patient was discharged on day 5 after transition to Orenitram. Patient will require close follow-up monitoring to assess any changes in clinical status.

Plan

Describe possible side effects of Orenitram and strategies for managing them

• Headache to be managed with medication
• Nausea to be managed with medication and by taking with food
• Diarrhea to be managed with medication

Counsel patient to notify the office immediately if any side effects become unmanageable or interfere with the ability to take Orenitram as prescribed

Schedule follow-up visits, including RHC for 6 months post-transition
